the cost for up to EIGHT massage
treatments per annum carried out by IMTA therapists.

you are a member of a VIVAS Day to
Day Scheme you can claim the relevant amount(1) of the cost of a
massage treatment on presentation of an invoice/receipt showing:
your
Name and VIVAS number, the date of the treatment, and
the
name and IMTA No. of the therapist providing the treatment.
